Please verify your Member Information is correct. If your name or mailing address is incorrect, please line out and write in the correct information.

*IMPORTANT* Your membership cannot be processed without answering and signing the survey. Please complete the survey before sending in your Membership Statement.

Candidates for the Board of Directors are entitled to mailing lists for election purposes as authorized by ALA Board policy. This policy only allows candidates to receive a list of members who have opted to be included on the list appropriate to their election race. A candidate for the North District would only receive a list of North District members who have opted to be included on the list. A candidate for an At-Large position would receive a list of all ALA members who have opted to be included on the list. You have the opportunity to choose whether or not to be included on these lists.

Signature: Please read the membership obligations at the bottom of page 1 and sign. Your statement cannot be processed without your signature.

Payment Information

1. Membership
The membership you selected last year has been filled in for you.

Slip Right (Dock) Membership: $500 annual fee. This membership fee is required of all dock slip and/or dock right/site owners. The DUE DATE is April 1, 2011. A late fee of $50 PER MONTH or any portion of a month will be due after June 1, 2011. Maximum late fees are $200 per dock or right and unpaid accounts will be presented to the Board of Directors.

This membership level included all the amenities of the Beach Club and General Memberships.

Beach Club Membership: $200 annual fee. This membership includes all the amenities of a General Membership plus access to the Tavern Bay and Burnt Mill Beach Clubs.

If you would like to choose a General Membership instead of a Beach Club Membership, please line out "Beach Club Membership" and "$200" and fill in "General Membership" and "$60."

General Membership: $60 annual fee. This membership entitles you to:
Boating on Lake Arrowhead (with ALA Boat Registration)
Access to over 60 lake trails
Use of Grass Valley Park and Peninsula Park
Ability to rent the Burnt Mill Beach Club building
Fishing
Voting privileges
Admission to special events

If you would like to choose a Beach Club Membership instead of a General Membership, please line out "General Membership" and "$60" and fill in "Beach Club Membership" and "$200."

Membership cards: If we have received your statement (with a signature at the bottom of page 1 and the Survey question answered and signed) and Membership payment, Membership cards will be mailed to you.

2. Boat Registration
A Membership purchase is required to register a boat(s).

The boats that are currently on ALA records are listed.

Insurance Expiration, DMV Expiration: Our records may show that your boat insurance or DMV may have expired. If so, there will be a note to "See Below" in the column. This notation means that ALA does not have a current copy of your boat insurance or DMV registration. Please include a current copy with your statement so we can process your boat decal without delay. Boat decals cannot be issued if ALA does not have current boat insurance or DMV information on file.

Dock # / Rental Dock #: Please fill in the dock number or rental dock number that each boat is "attached" to. If you are renting a dock for the 2011 season, please send in a current Dock Rental Agreement with your Membership Statement.

Storage Company: If you store your boat, please enter the name of the storage company for each boat. If you store with Hardin Marine or Mile High Marine your boat decal will be sent directly to those companies.

Fee: Fill in the correct annual fee for each boat that you would like to register. If you own a dock or are renting a dock your boat registration fee for a power or sail boat is $200 for the year. If you do not have a dock or are not renting a dock, the boat registration fee for a power or sail boat is $300 for the year. For all small non-powered boats (kayaks, canoes, sculls, pedal boats) the registration fee is $25 for the year. (Please review our Boating and Swimming Rules for ALA's boat size requirements.)

Decals: If we receive all the necessary information to process your boat registration, your boat decals will be mailed to you unless you store your boat with Hardin Marine or Mile High Marine or you contact the office and request your decals be held for pick up.

Boat not on the list? If you would like to register a boat that is not on the pre-printed list, fill in the information about the boat and send in copies of the following with your statement and payment:

Power/Sail Boats: Boat DMV Registration (the ALA Member must be on the title of the boat) and Boat Insurance ($500,000 liability with Arrowhead Lake Association named as Additional Insured).

Small Boats: Homeowner's Insurance declarations page.

We cannot issue boat decals without this information on file. If we have received all the necessary information, your boat decals will be mailed to you.

3. Boat Operator License
If your Boat Operator License has expired you may be able to renew your license without coming into the office. If you would like to obtain a new license you can follow the steps below but you must come into the office to have your photo taken for your license.

How to renew your Boat Operator License:

If you have already had your photograph taken for a Boat Operator License, you can follow these steps to renew your license without coming into the ALA office. Please do the following for each license you would like renew:

1. Complete the Boat Operator License test. Click here to view and print the Boat Operator License test. This test is an "open-book test." You can complete the test with the aid of our Boating and Swimming Rules. Click here to view the Boating and Swimming Rules while you complete the test.

2. You may send in your completed test with your statement and your payment of $15. Your Boat Operator License will then be mailed to you. Or, you may FAX your test to 909-337-6371. You may then call to pay the $15 fee with a credit card or send in the $15 fee with your payment. Your Boat Operator License will then be mailed to you.

If you are obtaining a new license you may send in your completed test and payment but you will have to come into the ALA office to have your picture taken before we can issue a Boat Operator License. When you come in you will receive your license immediately.

4. Donations
Donations to ALA are voluntary and greatly appreciated. You may donate to the 4th of July Celebration which will be held on Sunday, July 3rd this year. You may donate to the Fish Stocking which is done three times a year - twice in the spring and once in the fall. You may donate to a general Arrowhead Lake Association fund to help maintain the lake and shoreline.

5. Total Due
Please add your annual Membership fee, your annual Boat Registration for each boat, your Boat Operator License fee if you are renewing your license(s) and any donations to the Fireworks show, fish stocking or to ALA.

Please make your check payable to Arrowhead Lake Association or pay by credit card by calling the office at (909) 337-2595 or filling in your credit card information at the bottom of page 2.
